Brief specification description or basic overview of the project: Purchase physical examination services for specific populations in Nanhui New Town, including "Ivy" elderly health examinations and "Carnation" gynecological examinations. The winning bidder shall meet the relevant physical examination and information management needs of Party A, and share the physical examination data with the community health service center in this town.

合同履约期限：本项目一招三年，合同一年一签。首年服务期为自合同签订起至2025年10月01日。付款金额以当年度实际体检人数为准。

The Contract Period: This project has a three-year contract with a yearly signing. The first year of service period is from the signing of the contract to October 1, 2025. The payment amount shall be based on the actual number of physical examination personnel in the current year.

二、申请人的资格要求

2. Qualification Requirements for Bidder

(a)满足《中华人民共和国政府采购法》第二十二条规定；

(a)Meet the provisions of Article 22 of the "Government Procurement Law of the People's Republic of China";

(b)落实政府采购政策需满足的资格要求：（1）落实预留份额措施，提高小微企业在政府采购中的份额，扶持小微企业政策：本项目（□是 ■不是）专门面向中小微企业采购，评审时小微企业价格给予10%的扣除。（2）扶持残疾人福利性单位，并将其视同小微型企业；

(b)Qualification requirements to be met to implement government procurement policies: (1) Implement measures to reserve quotas, increase the share of small and micro enterprises in government procurement, and support policies for small and micro enterprises: This project (□ Yes ■ No) is specifically aimed at purchasing from small and micro enterprises, and a 10% deduction will be given to their prices during evaluation. (2) Support welfare units for people with disabilities and treat them as small and micro enterprises;

(c)本项目的特定资格要求：（1）本项目面向大、中、小、微型企业，事业法人、其他组织或自然人采购。
（2）法人依法设立的分支机构以自己的名义参与投标时，应提供依法登记的相关证明材料和由法人出具的授权其分支机构在其经营范围内参加政府采购活动并承担全部民事责任的书面授权。法人与其分支机构不得同时参与同一项目的采购活动。
（3）具有中华人民共和国医疗机构执业许可证。
（4）符合《关于对接国际高标准经贸规则推进试点地区政府采购改革的指导意见》（沪财采〔2024〕12号）第17条规定的供应商，不得参加本项目的采购活动。
（5）本项目不允许转包。

(c)Specific qualification requirements for this program: (1) This project is aimed at large, medium, small, and micro enterprises, public institutions, other organizations, or natural persons for procurement. (2) When a branch established by a legal entity participates in bidding in its own name, it shall provide relevant certification materials registered in accordance with the law and a written authorization issued by the legal entity authorizing its branch to participate in government procurement activities within its business scope and bear all civil liabilities. (3) Having a practicing license for medical institutions in the People's Republic of China. (4) Suppliers who comply with Article 17 of the Guiding Opinions on Promoting Government Procurement Reform in Pilot Areas in Connection with International High Standard Economic and Trade Rules (Hu Cai Cai Cai [2024] No. 12) are not allowed to participate in the procurement activities of this project. (5) This project does not allow subcontracting.

(i)符合《中华人民共和国政府采购法》第二十二条的规定；

(i)Comply with the provisions of Article 22 of the"Government Procurement Law of the People's Republic of China";

(ii)未被“信用中国”（www.creditchina.gov.cn）、中国政府采购网（www.ccgp.gov.cn）列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单；

(ii)Not included in the list of dishonest persons subject to enforcement, the list of parties to major tax violation cases, or the list of records of serious illegal and dishonestacts in government procurement by "Credit China" (www.creditchina.gov.cn), China Government Procurement Network (www.ccgp.gov.cn) ;
